Self referral form

You are currently completing a referral into the NHS Talking Therapies Team in Leeds.

This service offers structured therapy options for people struggling with symptoms of a common mental health problem such as anxiety, depression, obsessive compulsive disorder or post traumatic stress disorder, as well as others.

After you have completed this form, if your primary difficulty would be best managed by more appropriate specialist services, we may signpost you to those services straight away. If the interventions we offer may be suitable to meet your needs based on the symptoms you report on the form, you may be offered a treatment option straight away or you may be offered a follow-up call with one of our practitioners to discuss whether the interventions we offer in our service will be the right fit. We are unable to offer you treatment for one of the following reasons:

• You are not aged 17+ and/or registered with a Leeds GP. To find a GP, please visit the NHS website.
• Your responses to our questions have indicated that LMWS does not provide the most appropriate treatment for you. Please speak to your GP about what treatment may be suitable for you.
• You have already completed a referral to our service and are awaiting a response. Please continue with the referral as the most appropriate treatment for you will be offered in due course.
• You are currently engaging in (or on a waiting list for) one of our therapies (online, group, or face-to-face). Please continue with this process.

• Contact the NHS by calling NHS 111.

Existing diagnosis

If you have a diagnosis of obsessive compulsive disorder (OCD), post-traumatic stress disorder (PTSD), social anxiety or health anxiety – LMWS may not offer the recommended treatment for you if your condition is severe or complex. Please speak to your GP about the treatment options available.

If you are seeking support for physical symptoms of diabetes or advice regarding medication, please contact your GP, Leeds Programme Approach for Type 2 Diabetes, or Leeds Diabetes and Endocrinology for Type 1 Diabetes.
